Our vision plan

Humana customers have a choice of two vision plan options from EyeMed:

• A discount program available at no cost with Humana health and dental plans.
  
  
• Coverage for specific vision services as part of the health plan, in addition to the discount program.
  
  

Discount Program

Employers who purchase a Humana health benefits plan or HumanaDental plan receive the EyeMed vision discount program at no cost.

EyeMed offers discounts on eye exams, frames, lenses, and contacts received from providers in the EyeMed network. The network includes optometrists, ophthalmologists, opticians, and LensCrafters^® stores at 28,000 locations.

The vision discount program is not an insurance product.

Vision Riders

In addition to the discount program, customers have the option to enhance their Humana health benefits plan with optional vision riders. The riders provide coverage for specific vision care services. Vision riders are available with all Humana medical product lines.

Available riders include:

• Coverage for exam only
  
  
• Coverage for hardware only (frames, lenses, contacts)
  
  
• Coverage for both the exam and hardware
  
  
• PPO riders allow for coverage outside the EyeMed network
  
  
• HMO riders cover only participating EyeMed providers
