In addition, Humana Community Navigator is easy to use, delivering personalized results to your patient while reducing administrative burden to your practice.

To get started, visit humana.findhelp.com and enter your patient’s ZIP code. Then, type in a keyword or select a category to review resources specific to your patient’s needs.

Let’s consider a real-life scenario of a patient with food insecurity:

  • Type “food” into the search field and select from the drop-down or select the food category icon.
  • Review the results and choose the resource that best fits the situation.
  • After selecting a resource, you can provide a referral in the best way for your patient: written, printed, emailed or text.

Filters specific to the individual and program type can be applied to further narrow search results, ensuring the most relevant resources appear first. In addition, Humana Community Navigator can be translated into more than 100 languages, so you can filter and identify organizations that offer communications in different languages. Each resource can then be shared with the patient in their preferred language.
