- Annual vision exam
- Eyeglass fitting, correction and repair
- Eyeglasses, contact lenses and prosthetic eyes
- Testing, care and management of eye disease
- Vision exams for eyeglasses and contact lenses
Plus more from Humana Healthy Horizons in Florida, for members older than 21:
- One set of frames per year, or one six-month supply of contacts with a prescription (You will pay any cost that exceeds $75 for luxury frames)
