Humana’s NICU case managers provide telephone-based services for the parents of eligible infants admitted to a NICU.

Program overview

Case managers in the Humana NICU Case Management Program are registered nurses who help families understand the treatment premature babies receive while they are in the hospital and prepare to care for the infants at home. They engage families during and after the baby’s hospital stay. Case managers work closely with physicians and hospital staff to coordinate care during the infant's stay in the hospital and after discharge. They also help parents arrange for home health nurses, ventilators, oxygen, apnea monitors and other equipment and services needed to care for the infant at home.

After the infant is discharged from the hospital, nurses call the family to provide additional support.

Eligibility and cost

This program serves commercial, administrative-services-only (ASO) and select Medicaid members.

The program is available in all markets, except Puerto Rico. Referrals from any source are accepted. This program is part of eligible members’ medical benefit plans and is provided at no additional cost to the member.
