Caregiver support

Everyone needs a little help sometime. For our enrollees under age 17, that help may come from a parent or guardian. For our enrollees older than age 18, that help may come from another type of caregiver. We make information and resources available to the people caring for our enrollees, no matter their age.

Caregivers have an important role. They help loved ones maintain their health and well-being.

A caregiver may help with personal care, health and wellness, household chores, companionship, and safety checks. A caregiver may help their child—our enrollee—access the benefits that we offer our enrollees. This means that if you are the parent or guardian of a Humana Healthy Horizons in Kentucky enrollee who is 17 or younger, we can help you help your child make the most of his or her health plan.

Caregivers can help with many types of activities, including:

• Staying in touch with loved ones
• Managing finances
• Traveling to and from appointments
• Setting up a safe home
• Shopping for groceries and cooking
• Finding activities to do together

Share access with your caregiver

As a member, you can share access with a caregiver in different ways. This will give your caregiver access to your protected health information (PHI).

2 ways to give access:

• Online in MyHumana. Sign in now
• With a paper form. Download, complete, and mail the PHI consent form.
