Your health is important to us. Humana Healthy Horizons® in Kentucky enrollees can get support from a care manager.
Your care manager
If you qualify for support from a care manager, he or she can:
- Partner with you and your physician(s) to ensure you have the resources and education needed to manage your health
- Assist with scheduling tests and appointments
- Help you better understand what lab results mean and how to maintain or improve them
- Help you get medical supplies, if needed
- Help you develop and adhere to a meal plan and/or establish a healthy exercise routine, if needed
- Help you with any transportation needs, if needed
- Help make sure you take advantage of the extra programs, services, and healthy rewards included with your health plan
